Instructions for Submitting a Web-based Entry

Last Minute Entry Deadline: Friday, April 12, 2013
Late Entry Fee: $78 per entry
Note: Entries will not be accepted after 5PM PST on Monday, April 15th.

Please read the following information before completing an entry form.

  1. Complete all information requested on the entry form. English entries only.
  2. Your entry must be active throughout the judging period — April 22, 2013 through May 20, 2013. If your entry is not accessible during the judging period, it may not be reviewed.
  3. If necessary, provide us with access codes (user name/password). Access codes must be active throughout the judging period — April 22, 2013 through May 20, 2013. The codes you provide must allow up to four (4) judges to access your entry individually, and must not require a registration process. Our judges will not be asked to register or provide personal information during their reviews.
  4. You may submit as many entries as you'd like.

Submission Requirements:

E-Newsletters: Provide a direct URL to a single issue only. You may submit as many issues as you'd like, but only one (1) issue per entry.

RSS Feeds: Provide a direct link to your RSS feed. Our judges may also choose to use a feed reader.

Webcasts and Web-based audio and video: Provide us with a direct URL which will allow our judges to stream or download your audio/video.

Only English language entries will be accepted. If your entry is not in English, please provide us with a direct link to the English version.

Your entry must be active throughout the judging period —
April 22, 2013 through May 20, 2013.

For entries requiring access codes (user name/password), you must provide us with codes that will be valid throughout the judging period 4/22/2013 - 5/20/2013. If you can't provide a user name/password, you must provide us with a direct link to a demonstration video, Flash presentation, or PDF file with screen shots, so that our judges will be able to review your entry.

Payment: VISA, MasterCard, American Express, and checks are accepted. Make checks payable to the Health Information Resource Center (or HIRC). (Fed. Tax I.D. # 36-3559293)
